To Prevent Delays or Complications, ALL Medical History & Physical Forms:

• MUST be completed in ink, no e-signatures or exceptions
• Have ALL Contact Information
• Have the Grade Level of the PARTICIPATING Year
• Have the SCHOOL NAME at the top
• Have the STUDENT ID #
• Have the Medical History Questions answered by the PARENT
• Have any ‘YES’ answers on the Medical History Explained Thoroughly [use another paper if necessary]
• Have the signature of both the STUDENT & PARENT in INK (NO Electronic Signatures will be accepted)
• Should be delivered to the school, NOT scanned or photographed and emailed (send the paperwork along with the students since they will be here in person).
• MUST be submitted BEFORE any Try Outs/Practices in order to be eligible to participate. If there are issues with the paperwork, the student will be asked to leave the try out/practice until they become eligible to participate in UIL athletics
